Participation Agreement
Explanation of inherent risks, participation agreement and appropriate consent for participation in Martial Arts training.
(Please note Martial Arts include, but are not limited to the combat systems of: Jujitsu, Judo, Taihojutsu, Combat Sombo, Sombo Wrestling, Wrestling, Kobudo and other systems employing unarmed combat methods and weaponry usage for self defence, sporting or aesthetic purposes.)
Martial Arts training often involves forceful contact with the ground or other persons. Learning and practising a martial arts system frequently necessitates vigorous cardiovascular activity and stretching movements. Because of these conditions inherent to Martial Arts participating exposes a participant to many risks of injury. Those injuries include, but are not limited to, death; paralysis due to serious neck and back injuries; brain damage; damage to internal organs; serious injuries to the bones, ligaments, joints, and tendons; and general deterioration of health. Such injuries can result not only in temporary loss of function, but also in serious impairment of future physical, psychological, and social abilities, including the ability to earn a living.
In an effort to make martial arts training as safe as it can be, instructors will instruct participants concerning the rules of appropriate martial arts sub disciplines and the correct mechanics of all skills. It is vital that participants follow the Instructor’s skill instructions and advice to decrease the possibility of serious injury. Participants will also be provided with a safety brief that they must at all times adhere to.
I /we have read the above information concerning the risks of participating in Martial Arts. I/we and understand and agree to adhere to all instructions contained within the safety brief. I/we understand and assume all risks associated with training and competing in Martial Arts. I/we agree to hold all instructors, volunteers, participants and agents harmless in any and all liability actions, claims, or legal action in connection with participation in any activities related to participation in Martial arts training and competition.
In signing this form, I/we assume the inherent risks of Martial arts training and competition. I/we waive future legal action by my/our heirs, estate, administrator, assignees, family members and ourselves.
